This form is for independent contractors who want work from the desk. It is not a job offer. It is not a buyer request. Documents stay private. We do not publish a gallery of W-9s or certificates.

Who should apply

Independent contractors in lawn and snow, trash-out, cleaning, general repair, lock and board-up, roofing, plumbing, electrical, HVAC, structural repairs, foundation and masonry, crawlspace repairs, deck and porch repairs, carpentry and framing, flooring, drywall and painting, windows and doors, concrete and flatwork, fencing, waterproofing, restoration, rehab, and occupancy inspections. If you have employees, workers’ compensation belongs in the file. If you do not, use the no-employees attestation.

How work is offered

Coverage is a file. A market is staffed when we can put you on a property with insurance on file and a photo spec you will actually shoot. Applying does not guarantee volume. A match is a follow-up from the desk, not an automatic dispatch.

Public checklist

No GL = no live work order until a COI is on file. Never pressure anyone to work uninsured.

IRS Form W-9

UIR Realty Rehab is the requester of Form W-9. Use the official IRS form. Complete it, sign it under penalties of perjury, date it, and upload the signed PDF or image. Do not type a Social Security number or EIN into this website.

This is not a substitute Form W-9. IRS rules do not allow a substitute W-9 to mix unrelated contract terms into the IRS certifications. Download the current IRS form, complete that form, and upload it.

General liability certificate

No general-liability certificate means no live work order until a COI is on file. We will not ask you to work uninsured.

Workers’ compensation or no-employees attestation

If you have any employee subject to Kentucky’s Workers’ Compensation Act, upload a workers’ compensation COI. If you have no employees, complete the attestation below instead.

This attestation is a UIR vendor-file document. It is not a Kentucky government form, it does not change Kentucky law, and it is not legal advice. Kentucky requires every employer with one employee subject to the Act to secure workers’ compensation coverage.

No-employees attestation — complete only if you are not uploading a WC COI because you have no employees.

License screenshot or license-not-required attestation

Kentucky’s Department of Housing, Buildings and Construction licenses specific trades, including plumbing, electrical, HVAC, boiler, and sprinkler/fire-alarm work. Kentucky does not issue a statewide general-contractor license for all construction. Local permits can still apply. If the work you will perform for UIR requires a license, upload a current license screenshot. Do not use the attestation to cover licensed trades.
